PSAL Football Clinic

HSS has partnered with the Public Schools Athletic League (PSAL) to provide game-day medical coverage to selected teams and access to follow-up care in the event that game-related athletic injuries occur for all PSAL football teams.

HSS offers a weekly Monday clinic for the exclusive use of PSAL JV and varsity football athletes. If your player is injured on game day, have them treated by our orthopedic specialists within 48 hours.

Instructions

Coaches, parents or players may contact us at psalclinicpatient@hss.edu or by phone Monday-Friday at 646.797.8340 to make an appointment on game day after an injury occurs. If you email us by 12 noon on Monday (the day of the clinic), we would be happy to see your athlete.

The following forms must be completed in order for the athlete to be treated at PSAL Clinic:

  • Notification of Injury (pdf)
    Completed form must be signed by parent and coach / school, who is then responsible for sending directly to PSAL’s secondary insurer (AIG, Educational Markets). This form ensures that HSS is reimbursed by AIG, Educational Markets.
  • HSS General Consent Form (pdf)
    This form needs to be signed by the parent / legal guardian.
    If parent / legal guardian is not accompanying athlete to clinic, and HSS does not have the signed consent form, then the athlete cannot be treated
  • HSS Registration Form (pdf)
    This form may be completed upon arrival at the PSAL Clinic, but you may wish to begin completing in advance to ensure that you have all the necessary information with you.
  • If the parent / legal guardian accompanies the athlete, they must bring a legal form of identification (e.g., driver’s license, passport, etc.) to the appointment.

A Note on Insurance

The Public Schools Athletic League and Hospital for Special Surgery are committed to ensuring that PSAL athletes who sustain a game-related injury are able to receive affordable medical treatment. If you are a PSAL athlete, you are able to come to HSS for treatment and will not be responsible for payment of bills resulting from the treatment of game-related injury. The services provided by HSS in the treatment of game-related injury may be covered by employer-based commercial health insurance plans. However, if for any reason your insurance company denies coverage, or if you have health insurance coverage through Medicaid or no health insurance coverage, the PSAL and HSS have partnered with Maksin – a secondary insurance provider – to cover the cost of medical treatment for game-related injury for PSAL athletes.
